I HEREBY ASSUME ALL RISKS ASSOCIATED WITH PARTICIPATING IN THE EVENT ON ______________ (date) AT CAMP CURIOSITY, CURIOSITY ACADEMY & DOYLESTOWN PICKLEBALL AND ANY AND ALL ACTIVITIES ASSOCIATED WITH THIS EVENT. I ASSUME ALL RISKS including by way of example, and not limitation, any risks that may arise from the negligence or carelessness on the part of Camp Curiosity, Inc., The Curiosity Shoppe, Inc., The Toddler Center, Inc., and Doylestown Pickleball (herein, collectively, “Curiosity”) from dangerous or defective equipment or property owned, maintained or controlled by Curiosity.
I certify that I am physically fit, have sufficiently prepared for participation in this activity, and have not been advised not to participate in this activity by any medical professional. I certify that there are no health-related reasons or problems that would preclude me from safely participating in this activity.
In consideration of my participation in the activities at Curiosity, I hereby take the following action on my own behalf, or on the behalf of a minor child, and on behalf of my executors, administrators, heirs, next of kin, successors, and assigns:
I WAIVE, RELEASE, AND DISCHARGE Curiosity, its owners, employees, officers, directors, volunteers, representatives, and agents, from any and all liability, including liability arising from the negligence or fault of Curiosity, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me.
I hereby INDEMNIFY AND HOLD HARMLESS, AND PROMISE NOT TO FILE SUIT AGAINST Curiosity, its owners, employees, officers, directors, volunteers, representatives, and agents for any and all liabilities or claims made as a result of participation in activities while at Curiosity.
This Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under the laws of the Commonwealth of Pennsylvania.
I HEREBY CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENTS. I HAVE SIGNED THIS DOCUMENT OF MY OWN FREE WILL AND I AM AWARE THAT THIS IS A RELEASE OF LIABILITY.
